Getting your Level 2 Fingerprint Background Check in Jax Florida, Fingerprinting is quick and straightforward. Here’s how it works:

  • 1. Employer Registration (AHCA Clearinghouse)

    Your hiring agency or licensing organization must first register you in the AHCA Clearinghouse through the Agency for Health Care Administration system and generate your Live Scan Request Form. This form includes your ORI number and Screening ID.

  • 2. Bring Required Documents

    Bring your Live Scan Request Form (CHAI) along with a valid, unexpired photo ID (driver’s license, state ID, or passport) to our Jacksonville fingerprinting location.

  • 3. Live Scan Submission

    We capture your fingerprints digitally and securely submit them through the Florida Department of Law Enforcement, which forwards them to the Federal Bureau of Investigation for national background screening.
