  • 2. Bring Required Documents
    Bring your LiveScan Request Form (CHAI or APD screening form) along with a valid, unexpired photo ID (driver’s license, state ID, or passport) to your fingerprinting appointment.

  • 3.LiveScan Submission

    Your fingerprints are captured and securely submitted by Jax Florida Fingerprinting to the Florida Department of Law Enforcement (FDLE) through the Care Provider Background Screening Clearinghouse.

  • 4. Clearinghouse Processing

    Your submission is processed through the Florida Background Screening Clearinghouse, where APD reviews eligibility for employment or provider approval.
